Speaker Bio:
Dr. Cohen grew up in New York City and obtained his undergraduate degree in Biology and Classics at Brown University where he was also a Division I athlete. He graduated from the combined MD/PhD program at the Icahn School of Medicine at Mount Sinai, earning his PhD in the laboratory of Domenico Tortorella, PhD studying cytomegalovirus infection to characterize potential new therapeutic targets and drugs. He completed his residency in Pathology at New York-Presbyterian/Weill Cornell Medical Center where he served as Chief Resident in Clinical Pathology followed by a fellowship in Blood Banking/Transfusion Medicine at the New York Blood Center and New York-Presbyterian/Columbia University Irving Medical Center. He is on the faculty of Weill Cornell Medicine as Assistant Director of Transfusion Medicine and Cellular Biotherapies and Assistant Professor of Pathology and Laboratory Medicine. He also serves as the Transfusion Medicine Consultant for the Bone Marrow Transplant program and signs out cases for the Special Hematology and Coagulation Laboratory. Dr. Cohen volunteers with the Association for the Advancement of Blood and Biotherapies (AABB) on the Transfusion Transmitted Diseases committee and serves as Treasurer on the Board of Directors for the Blood Bank Association of New York State (BBANYS).

Abstract:

Post-partum hemorrhage (PPH) remains a leading cause of maternal morbidity and mortality, affecting 3–5% of deliveries in the United States. Current transfusion protocols often treat PPH as either trauma or non-trauma, despite pregnancy’s unique hemostatic physiology. This talk reviews the hematologic changes during pregnancy, current ACOG and FIGO guidelines, and emerging evidence on massive hemorrhage protocols tailored for obstetric patients. We will explore the role of early fibrinogen replacement, goal-directed therapy using viscoelastic testing, and institutional considerations for developing PPH-specific protocols. Attendees will gain insight into optimizing transfusion strategies to improve outcomes in this critical setting.

Objectives:

1. Describe the basic physiologic changes in hemostasis and thrombosis during pregnancy.
2. Discuss the current recommendations for transfusion support for post-partum hemorrhage.
3. Evaluate whether post-partum hemorrhage requires its own massive hemorrhage protocol at your institution.
