Speaker Bio:
Dr. Tayler Van Denakker is currently assistant professor of pathology and assistant medical director of the blood bank and transfusion services at Mount Sinai Hospital in New York. She completed her residency in anatomic and clinical pathology at Mount Sinai followed by fellowships in hematopathology at Weill-Cornell Medical Center and transfusion medicine at Johns Hopkins Hospital.

Abstract:
Transfusion of patients with red cell antibodies poses significant clinical and laboratory challenges requiring individualized decision-making. This presentation reviews the immunologic mechanisms of alloimmunization, including antigen- and recipient-related risk factors, and examines the clinical consequences of antibody formation. Case-based examples highlight delayed hemolytic transfusion reactions, hyperhemolysis syndrome, hemolytic disease of the fetus and newborn, and passenger lymphocyte syndrome. Special populations such as patients with sickle cell disease, pregnant patients with alloantibodies, individuals receiving anti-CD38 therapy, and those with rare or high-frequency antigen antibodies are discussed. Practical strategies for blood selection, transfusion risk mitigation, and alternatives to transfusion are emphasized.

  3. Understand unique transfusion considerations in sickle cell disease.
